School Doctor Appointments for Summer Weight Lifting and Fall Sports
The school doctor will be here every other Wednesday until June 1.  If you would like to make an appointment to see the school doctor you  may sign up a  Here.  Please make sure to complete all necessary paperwork prior to physical date.  No one will be allowed to go into the weight room over the summer unless they have a current physical on file. 

Summer Sports Physicals

The school doctor will be here on Tuesday, June 21st  8:00- 12:00 for sports physicals. You may sign up Here.  Please remember to complete all necessary paperwork and registration before arriving to your appointment time. 


Sports Paperwork

Required in order to be cleared to participate:
1. Physical completed by your doctor on the state-mandated forms.
     Please make sure all forms are filled out completely, they are signed by the doctor and have the doctor's stamp. 
2. ImPact test completed  for all 9th and 11th graders. 
3.  All athletes need to be registered on the R School Registration Site
4. If your child has asthma, allergies, or diabetes a health plan will need to be submitted. All forms are listed below:

Over-the -Counter Medication Administration

The school nurse, according to the West Morris Regional High School District standing orders, may administer designated  during school hours. Over the Counter Medication Consent Form For the students' convenience, we stock regular-strength Tylenol (Acetaminophen), Advil (Ibuprofen), and Benadryl (Diphenhydramine),
