Prior to submitting your request for facilities use, please review Board policy and regulation 1330, Use of Facilities, including references to policies 1230, School Connected Organizations; 3514 Equipment; 3515, Smoking Prohibition; and, 6145 Extracurricular Activities.

I/We have read and agree to the terms as set forth in the West Morris Regional High School District's Board Policies and Regulations.

I/We understand that by completing the Use of Facilities form there is no guarantee the request will be approved.

**Due to Covid-19 and restrictions currently in place, there are no guarantees for facilities use requests in the 2020-2021 school year.
